Tottenham GP project


This is a pilot project via a London Primary Care Trust to integrate a GP’s clinical system (EMIS PCS) to a mobile device to assist the GP on his visits to the elderly care home to monitor and manage the elderly and their various chronic ailments.



The GP’s laptop will allow access to clinical information of the individual patient during the home visits allowing the GP to follow up on all chronic disease management, follow up on reminders & alerts and take immediate action as required. Such procedures will not only reduce drastically the administrative time of the GP (one entry multiple use of the clinical data) but enhance the GP’s clinical time spent much more efficiently.


A hand held mobile device, to be used by the Community Nurse Manager, will be developed in parallel to the GP’s laptop, to mimic similar clinical and admin activities.


Elderly Care Management


This pilot project has the potentiality of expanding to a number of larger practices to enable Practice Based Commissioning (PBC) to the Group of GPs under the Trust. The PBC group is looking to set up Specialists Clinics within the GPs in the group, to monitor the various chronic disease patients of the overall population. Such Clinics will include at risk patients and help reduce emergency situations by providing preventative care and efficient monitoring.



Similar Clinics have been initiated in other areas with manual workflow. With CAL2CAL’s support these Specialist GPs will have the added advantage of mobile access to the individual patient’s records to enable a full clinical analysis and clinical intervention. The Integration System enables intelligent reporting and monitoring thus providing an even productive way of managing the patient population with better care and immediate management. The system is anticipated to increase huge saving to the PBC budget for the Group by decreasing hospital visits and admissions of the patients with the in-house care and monitoring.
